Previous Years Forms
Certificate of Veterinary Inspection: All animals exhibiting during the Converse County Fair must have a current Certificate of Veterinary Inspection prior to unloading animals. Crossroads Veterinary Clinic, Riverbend Veterinary Hospital, Advanced Animal Clinic, and Smylie Animal Clinic are making time in their schedules for 4-H and FFA animal checks, the week prior to fair. These dates will be posted on Facebook. The veterinarians have asked that exhibitors complete the Certificate of Veterinary Inspection as much as possible before you go to their clinic for the inspection. If you plan to bring your dog and have it in the campground area, you will need to have a current health on it as well. (please do not bring dogs into the livestock barn)

2021 Market Sale Agreement - Required for all participant selling in the 2021 Youth Market Sale - Member must attend mandatory Market Sale Meeting on Monday, June 14, 6 PM at the Douglas High School Ag Shop (Door 10)
2021 Market Animal Drug-Free Verification This form must be completed and returned to the Converse County Fair Board Manager by June 14, 2021.
